Hong Kong's Hospital Authority published its 83rd Risk Alert quarterly report, detailing eight serious medical risk incidents in public hospitals from April to June 2026, including five cases of retained surgical items.
Hong Kong's Hospital Authority (HA) on 30 October published the 83rd edition of its quarterly Risk Alert bulletin, aimed at strengthening the reporting and monitoring of sentinel medical risk events in public hospitals to improve safety standards.
The latest bulletin lists eight serious risk incidents reported by public hospitals for the second quarter of 2026 (April to June). These include five cases of surgical instruments or medical materials left inside patients after surgery or interventional procedures, one medication error, and two cases classified under other categories.
In addition, 21 medication-related incidents were reported under the category of important risk events, the authority said.
An HA spokesperson said: “The Hospital Authority has conducted in-depth reviews and root-cause analyses of these incidents, drawing valuable lessons on patient safety. Through this publication, we share the cases and related improvement recommendations with healthcare staff to prevent recurrence of similar incidents.”
The HA headquarters has distributed the latest edition to staff. The public can access the bulletin on the HA website at www.ha.org.hk/riskalert. The Risk Alert is published quarterly in January, April, July and October. The next edition is scheduled for January 2027.
